The difference comes down to two things: how much pressure is missing, and whether the tire is leaking actively. A tire that “just needs air” is usually 5–15 PSI below spec, looks slightly low but still round, and holds pressure once you fill it. A truly flat tire is more than 50% below spec, often deformed where it meets the ground, sometimes shows a visible puncture, and starts losing pressure again within minutes or hours of being topped off.
The action you take depends on which one you have. A tire that needs air gets filled at any compressor and you’re on your way. A flat tire needs a plug, patch, or replacement — and if you keep driving on it without realizing, you can permanently damage the tire’s internal structure or the wheel itself.

The 4-Step Diagnosis
- 1. Look at the tire. Compare to the others on the same axle. If it’s visibly squished where it meets the ground (a wider, flatter contact patch than the others), it’s significantly low. If the sidewall is bulging out near the ground, it’s flat.
- 2. Check pressure with a gauge. Anything within 5 PSI of spec just needs air. Anything more than 50% below spec is functionally flat — don’t drive on it without filling first.
- 3. Look for the puncture. A nail, screw, or sharp object stuck in the tread is the most common cause of slow leaks. Walk the full circumference of the tire looking at the tread surface.
- 4. Fill and re-check. If you fill to spec and the tire holds pressure for 30+ minutes, you just needed air. If pressure drops noticeably within an hour, you have an active leak that needs professional repair.
“Just Needs Air” Indicators
- Pressure is within 15 PSI of spec. Typical numbers: tire reads 25 PSI when spec is 32 PSI. Low but not flat.
- Tire holds its shape, with only slight visible bulge at the ground.
- No visible puncture when you walk the tread.
- Holds pressure after filling. Top up to spec, check again in 30 minutes — same reading. Check again the next morning — same reading.
- Often coincides with a major temperature drop. First cold snap of autumn drops pressure 4–5 PSI; that’s enough to read low and trigger TPMS without anything actually being wrong with the tire.
If all five apply, you just needed air. Top up to spec, drive normally, and check pressure again in a few days to confirm it’s holding.
“Actually Flat” Indicators
- Pressure is below 50% of spec (under ~16 PSI on a tire spec’d at 32 PSI), or the gauge reads 0.
- Visible deformation — the bottom of the tire is squashed, the sidewall bulges outward, or the rim is sitting noticeably closer to the ground than the other tires.
- Visible puncture, gash, or sidewall damage when you inspect.
- Audible hissing when you crouch close to the tire, especially around the valve stem or where a nail might be embedded.
- Doesn’t hold pressure after filling — you top up to spec and within minutes or hours, the pressure has dropped significantly.
If any of these apply, treat it as a flat tire. The remedy depends on severity (see below).
What to Do for Each Case
Just Needs Air
Fill at the nearest gas station, parking lot air pump, or home compressor. Top up to the door-jamb spec (the figure on the sticker inside the driver’s door, not the sidewall max). Re-check pressure the next morning when the tire is cold to confirm it held.
If pressure drops 3+ PSI overnight, you have a slow leak even though it didn’t look like a flat tire. Schedule a tire shop visit for inspection and patch ($25–$40).
Flat but Drivable to a Shop
If the tire is significantly low (under 50% of spec) but you can still fill it and it holds for at least 30 minutes, you can usually drive a short distance (under 5 miles, under 35 mph) to a tire shop for repair. Avoid highways and high-speed driving — a underinflated tire at speed builds heat fast and can fail catastrophically.
If the tire won’t hold pressure long enough to drive to a shop, switch to your spare or call a tow.
Completely Flat or Visibly Damaged
Don’t drive on it at all if you can avoid it. Even short distances on a fully flat tire damage the sidewall against the rim and can permanently bend the wheel. Options:
- Change to the spare if you have one and know how. Most modern cars have a compact spare that’s good for 50–70 miles at 50 mph to get you to a shop.
- Call roadside assistance if you have it (AAA, insurance roadside coverage, or your vehicle’s bundled assistance). They can either change the tire for you or tow you to a shop.
- Call a mobile tire service (most metro areas have at least one) for an on-site change or temporary plug.
- Don’t try to drive on a run-flat tire below the manufacturer’s max speed/distance — usually 50 mph for 50 miles. Run-flats are designed to handle a flat temporarily but they have specific limits printed in your owner’s manual.
When “Just Needs Air” Is Actually a Slow Leak
The tricky case: a tire that loses pressure slowly enough that it looks “just low” every time you check, but actually has a small leak you keep papering over by topping up. Signs:
- You find yourself topping off the same tire repeatedly. Normal permeation loses 1–2 PSI per month per tire. If one tire needs 5+ PSI added every few weeks, you have a leak.
- The TPMS light comes on shortly after you top up. Suggests the tire is losing pressure within days rather than weeks.
- One tire reads significantly different from the others after a long parking period (days).
Slow leaks are usually caused by a nail or screw in the tread (often invisible because it sealed itself with the puncture object still in place), a corroded rim where the tire bead seals, or a failing valve stem. All three are repairable for $25–$50 at a tire shop.
